Death Claims Requirements

1. Completely filled, signed and stamped death claim form

2. Valid copy of the life insured’s ID

3. Original death certificate 

4. Official document on cause of death

5. Final detailed hospital report (If death occurred in a hospital)

6. Police report and Final General Attorney report (If death occurred due to an accident)

7. Stamped proof of employment stating the employment date and last working day date
